Operational failures in military and emergency contexts are routinely attributed to technical malfunction, inadequate training, or weak leadership. What such accounts tend to miss are the subtler psychological and organizational processes that wear down collective judgment long before a crisis unfolds. Cognitive bias, hierarchical communication patterns, and group dynamics accumulate quietly, surfacing only when the cost of error is highest. Drawing on decision science, organizational theory, and documented case failures, this article develops a multi-level framework for understanding how individual biases, organizational sensemaking, and conformity pressures interact to amplify error under stress. The central argument is not that failures can be prevented through better procedures or stronger leadership, but rather that error is an intrinsic property of complex decision systems, one that must be designed around, rather than designed out. The analysis questions the limits of technical and procedural fixes and makes the case for organizational structures and leadership practices built to remain functional in the presence of human fallibility. Implications are drawn for military training, leadership development, and decision-support system design.
