ABSTRACT Background and Aims Hypertension (HTN) is a major non‐communicable disease and a leading risk factor for cardiovascular morbidity and mortality globally. In Bangladesh, regional disparities in HTN prevalence and risk factors remain underexplored. This study aimed to assess the prevalence of HTN and its associated risk factors among adults in Dinajpur district, located in the northern region of Bangladesh. Methods A cross‐sectional study was conducted from December 10, 2024, and February 10, 2025. Adults aged ≥ 30 years with a documented blood pressure measurement from a certified diagnostic center in the preceding month were included. Data were collected through face‐to‐face interviews using a structured and pilot‐tested questionnaire. Data quality was ensured by checking for missing values and multicollinearity. Descriptive statistics summarized participant characteristics. Bivariate associations with HTN were assessed using χ2 tests and variables with p < 0.05 were included in multivariable logistic regression to identify independent predictors. Model performance was evaluated using the receiver operating characteristic (ROC) curve. Results Among 1026 participants, the overall prevalence of HTN was 36.5%, exceeding the national average. Significant predictors included age 61–70 years (AOR = 10.92; 95% CI: 5.19–22.99), being a housewife (AOR = 2.59; 95% CI: 1.12–5.83), positive family history of HTN (AOR = 1.96; 95% CI: 1.44–2.67), diabetes (AOR = 2.17; 95% CI: 1.49–3.18), kidney disease (AOR = 2.18; 95% CI: 1.20–4.26), overweight (AOR = 2.35; 95% CI: 1.29–4.27), and obesity (AOR = 1.95; 95% CI: 1.41–2.70). Lower family income (< 20,000 BDT) was associated with higher HTN odds (AOR = 0.66; 95% CI: 0.47–0.93). The model showed good discriminatory ability (AUC = 0.77). Conclusion HTN is highly prevalent among adults in Dinajpur District, driven by both modifiable and non‐modifiable risk factors. Region‐specific interventions focusing on early detection, lifestyle modification, and integrated management of comorbidities are needed to reduce the HTN burden in this underserved population.
