Penangan Pasien dengan Riwayat Stroke dengan Gigi Tiruan Lengkap Overdenture

Latar belakang. Pada kondisi sistemik tertentu yang tidak memungkinkan untuk dilakukan pencabutan seperti pada pasien dengan riwayat stroke perawatan dengan overdenture merupakan alternatif yang paling tepat. Overdenture adalah gigi tiruan lengkap atau sebagian yang didukung oleh mucoperiostium dan beberapa gigi atau akar gigi asli yang telah dilakukan perawatan saluran akar, untuk menghambat proses resobsi tulang alveolaris sehingga retensi dan stabilisasi gigi tiruan lengkap dapat ditingkatkan. Tujuan. Laporan kasus ini untuk mengetahui penanganan pasien dengan riwayat stroke dengan Gigi Tiruan Lengkap (GTL) overdenture. Kasus. Pasien wanita, 58 tahun dating atas kemauan sendiri ke RSGM Prof. Soedomo Fakultas Kedokteran Gigi Universitas Gadjah Mada, ingin dibuatkan gigi tiruan rahang atas dan rahang bawah, karena gigi tiruan yang dipakai sebelumnya sudah tidak nyaman diapakai mengunyah, pasien dengan riwayat stroke, saat ini masih dalam perawatan. Gigi yang masih tinggal adalah gigi 28, gigi 42 dan gigi 43, oleh karena merupakan kontra indikasi pencabutan, maka gigi yang masih tinggal tidak dicabut dan direncanakan sebagai gigi penyangga GTL overdenture dengan kaitan coping pada gigi 28, kaitan magnet pada gigi 42 dan base root pada gigi 43, selanjutnta diinsersikan GTL overdenture rahang atas dan rahang bawah, yang diperiksa: retensi, stabilisasi, oklusi, estetika dan fonetik. Kontrol dilakukan 1 minggu kemudian diperiksa keluhan pasien saat memakai GTL: pada pemeriksaan subyektif dan obyektif, diperiksa retensi, stabilisasi, oklusi, estetik dan fonetik. Hasil. Perawatan GTL overdenture dengan menggunakan gigi yang masih tinggal dengan kombinasi penyangga overdenture pada pemeriksaan subyektif pasien merasa lebih nyaman, pada pemeriksaan obyektif overdenture dapat meningkatkan retensi, stabilisasi, oklusi, estetika, fonetik. Kesimpulan. Pemakaian GTL overdenture dapat meningkatkan retensi, stabilisasi, oklusi, estetika dan fonetik. Overdenture merupakan perawatan pilihan untuk pasien dengan keadaan sistemik tertentu yang tidak mungkin dilakukan pencabutan.

The patient was a woman, 58 years old to come on their own to the Hospital Soedomo Faculty of Dentistry, University of Gadjah Mada, want to made a denture maxilla and mandible, as previously worn dentures that are nor comfortable to wear chew, patients with a history of stroke, while still in treatment. Teeth that remain are 28 teeth, tooth 42 and tooth 43, is contraindicated because of the revocation, the teeth that are left are not revoked and planned as CD overdenture abutment coping with regard to the teeth 28, the magnetic connection of the teeth 42 and the base root on tooth 43, the next is inserted CD overdenture maxillary and lower jaw, which examined: retention stabilization, occlusion, aesthetics and phonetics. Controls performed 1 week later examined patient complaints when using CD the subjective and objective examination, examined retention, stabilization, occlusion, esthetics and phonetics. Results. CD overdenture treatment using gear that is still living with the combination of buffer overdenture on a subjective examination of the patient feel more comfortable, the overdenture objective examination can improve retention, stabilization, occlusion, esthetics, phonetics. Conclusion. CD overdenture usage can increase retention, stabilization, occlusion, aesthetics, and phonetics. Overdenture is the treatment of choice for patients with certain systemic conditions that are nor possible revocation.
