Purpose: To assess the viability of the peroneus longus tendon allograft (PLTA) as a suitable alternative to traditional allografts used in anterior cruciate ligament reconstruction (ACLR). Methods: Surgery center logs were used to identify patients who underwent ACLR with PLTA between January 2011 and January 2017 with at least 1 year of follow up. Patient demographics such as age, gender, surgical history, and presence of meniscus pathology were collected in addition to allograft data including donor age and sex. Results of both objective and subjective outcomes measures – IKDC & modified Lysholm knee score, single leg hop test, and KT-1000 evaluation – were compiled and evaluated at final follow up. Results: Data for 28 patients met the minimum follow up. The mean age was 35.8 years. Fourteen out of the 28 patients were female (50 %) and 13 had meniscus pathology addressed at index operation. Mean postoperative follow-up was 52 months (range 13–161). The mean age of the allograft donor was 46.4 (range 22–68). The average IKDC score was 93.05 (SD 19±16.25). The mean modified Lysholm score was 95.0 (SD±9.25). Single-leg hop results averaged 94.4 % (SD±8.2 %, range 80–110 %) of the non-operative extremity. The mean KT-1000 arthrometer measurement was 0.32 mm, 0.4 mm, and 0.6 mm for 15, 20, and 30lbs of resistance, respectively. No patient required reoperation. Conclusion: In the appropriate patient population, ACLR with PLTA can be a clinically successful operation based on patient reported outcome measures, clinical outcome measures, and graft failure rates.
