Los macrólidos son un grupo de antibacterianos caracterizados por la presencia de un anillo lactona macrocíclico como núcleo de su estructura química. Son agentes bacteriostáticos, aunque en determinados casos pueden tener acción bactericida. Presentan buena actividad frente a cocos aerobios grampositivos, algunos aerobios gramnegativos y ciertos microorganismos anaerobios. Los nuevos compuestos poseen mayor actividad contra bacterias "atípicas" e incluso espiramicina es activa frente a algunos protozoarios. En el pasado la resistencia frente a macrólidos fue poco relevante, sin embargo en la actualidad la situación es diferente y el estudio de mecanismos de resistencia ha cobrado mayor importancia. El perfil farmacocinético de estos compuestos se caracteriza por concentraciones plasmáticas relativamente bajas y tisulares altas. La integración farmacocinética/farmacodinámica (PK/PD) es un pilar fundamental para el diseño de planes terapéuticos racionales y para ello se debe conocer cuales son los parámetros a considerar en cada caso. En este artículo se hace una revisión de la estructura química y clasificación de los diferentes compuestos del grupo, espectro y mecanismo de acción, resistencia antibacteriana, farmacocinética y farmacodinamia, interacciones medicamentosas, toxicidad y efecto antiinflamatorio. Macrolides are a group of antibacterials characterized by the presence of a macrolide lactone ring, as the center of its chemical structure. They are bacteriostatic antibiotics; but in certain cases they can act as bactericidal compounds. They show good antibacterial activity against aerobic grampositive cocci, some aerobic gramnegative organisms and certain anaerobic microorganisms. New macrolides have a particular action against "atypical" bacteria and in some cases spiramycin acts as an antiprotozoal drug. In the past, resistance was considered almost irrelevant, nevertheless the present situation is different and studies about resistance are very important. The pharmacokinetic profile is characterized by low plasma concentrations but high tissue ones. Pharmacokinetic/pharmacodynamic integration (PK/PD) is the base for the design of rational therapeutic protocols so that is necessary to know the parameters that must be considered in each case. This article presents a revision of the chemical structure and classification of macrolides drugs, spectrum and mechanism of action, antibacterial resistance, pharmacokinetics and pharmacodynamic behaviour, drug interactions, toxicity and anti-inflammatory effect.
